Wellness Director---**Job Overview:**We are seeking a compassionate
and dedicated Care Partner to join our team. The Care Partner plays
a pivotal role in providing high-quality care and support to
individuals in need of assistance due to physical, emotional, or
cognitive challenges. This position is ideal for someone who is

delivering personalized care in a respectful and supportive
environment.---**Key Responsibilities:**- **Personal Care
Assistance:** Provide support with daily living activities,
including bathing, grooming, dressing, and toileting, ensuring the
dignity and comfort of individuals in your care.- **Emotional
Support:** Establish and maintain positive, trusting relationships
with clients, offering companionship and emotional support to
enhance their quality of life.- **Medication Management:** Assist
clients in managing their medications as prescribed, ensuring
accurate understanding and adherence to medication schedules.-
**Mobility Assistance:** Help clients with mobility needs,
including transferring, walking, and using assistive devices, to
promote independence and safety.- **Household Support:** Perform
light housekeeping duties, including cleaning, laundry, meal
preparation, and grocery shopping, to maintain a safe and
comfortable living environment.- **Communication:** Serve as a
liaison between clients, families, and healthcare providers,
effectively communicating any changes in health or behavior to the
appropriate parties.- **Documentation:** Maintain accurate and
timely records of care provided, client progress, and any concerns
or observations, ensuring compliance with organizational policies
and regulations.- **Team Collaboration:** Work collaboratively with
other care team members, attending training and meetings as
required to ensure continuous improvement in care
delivery.---**Qualifications:**- **Education:** High school diploma
or equivalent preferred. Completion of a certified nursing
assistant (CNA) program or relevant training is a plus.-
**Experience:** Previous experience in a caregiving role, such as
home health aide, nursing assistant, or similar position, is
preferred.- **Skills:**
- Strong interpersonal and communication skills to engage with
clients and families effectively.
- Empathy and understanding of the needs of individuals with
varying levels of care requirements.
- Ability to handle potentially stressful situations calmly and
professionally.- **Certifications:** CPR and First Aid
certification preferred. Additional training in specialized care
(e.g., dementia, hospice, or physical disabilities) is
advantageous.- **Physical Requirements:** Ability to perform
physical tasks, including lifting and transferring clients,
standing for extended periods, and assisting with
